Your First Visit

patient information

Ideally your initial appointment will consist of a consultation explaining your diagnosis and treatment options. However, we also appreciate that your time is valuable.  For that reason, we will do our best to accommodate you for some treatment the same day of your initial appointment.  Depending on your needs and the condition of your oral health there may not be enough time to complete your cleaning at the same appointment. A complex medical history or treatment plan will likely require more evaluation and subsequent appointments.

Please assist us by providing the following information at the time of your consultation:

  • Current x-rays (if available)
  • A list of your current medications prescription and non-prescription.
  • If you have dental insurance, bring the necessary policy numbers.  This will save time and allow us to help you process any claims.

IMPORTANT: A parent or guardian must accompany all patients under 18 at the initial visit.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication that may directly affect treatment that day(i.e. heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If your previous dentist has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
